Our health services benefit the individual, family, and community by integrating primary care, dental care, WIC support, mental health counseling, community health education, and more under one roof.
We have primary care facilities in Union City, Hayward, and San Leandro and several program sites in Alameda County, California.

The Family Support Services Case Management Program provides comprehensive case management services to pregnant. And also parenting teens and young adults. Eligible clients are pregnant under the age of 25 or have children under the age of 5. Ancillary services are free, voluntary, And also confidential.

Community health education programs use peer-to-peer health promotion to raise awareness. And also lead to healthy lifestyles and positive social change. Our Promotoras de Salud volunteers become leaders, health educators, advocates, and change agents in their communities.
Health Center Of Optometry
Our eye health professionals provide eye care, diagnosis, and treatment, emphasizing comprehensive care and education. And also prevention. Services include routine eye exams, ophthalmic diagnosis and treatment, pre-and post-surgery care, low vision services, eyeglasses, and other vision-enhancing prescription services.
